To request your high school transcript, please send an email request with the below information to Ashley.Baker@CarlisleIndians.org.  Please note that the school does not keep copies of high school diplomas.

• First and Last name while attending Carlisle High School
  •    Current last name if it has changed
• Birth date
• Graduation year or last year of attendance
• The method you would like to receive your transcript   
  
  • Mail - provide complete mailing address
  • Email - provide email address
  • Pick up in person - provide current phone number to receive a call when it's ready
  • Fax - provide fax number
