Enrollment Information

If you live within the Carlisle Local School District, please use the information below to register your student(s).

If you do not live within the Carlisle Local School District, please Click Here for open enrollment information.
Please note: open enrollment applications must be completed and approved before registering your student(s).

Student Registration

Students enrolling or re-enrolling at Carlisle Local Schools must use FinalForms to complete the registration process. To begin registration:

  1. Click Here to go to FinalForms
  2. Locate the Parent icon and click NEW ACCOUNT, then follow the steps to create your account.
  3. Type your First Name, Last Name, Date of Birth, and Email. Next, click REGISTER.
  4. Check your email for an Account Confirmation Email from the FinalForms Mailman. Once received and opened, click Confirm Your Account in the email text.
  5. Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
  6. Click REGISTER STUDENT for your first child.


The following forms/information are required to complete registration:

  •          Birth Certificate
  •          Social Security Card
  •          Parent Driver's License
  •          Proof of residency (mortgage statement/deed or rental agreement)
  •          Proof of custody if applicable
  •          Immunization record
  •          IEP/MFE if applicable


Once you have completed the online forms, you will be contacted by the school to confirm all documents/information were received and to notify you when your child is able to start attending.
